Julie Lemire

Julie is a bilingual registered Speech-Language Pathologist and Rehabilitation Case Manager with over 22 years of experience working as a multidisciplinary team member in acute care hospitals, community-based rehabilitation, and private practice.   She has also been on the board of directors at OSLA and guest lecturer at the University of Ottawa.  Over the years, Julie has developed an extensive network of community resources which she uses to assist clients in maximizing their recovery.  Julie’s empathy and interpersonal skills allow her to connect well with her pediatric and adult clients.

Julie is a member in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario (CASLPO) and Speech-Language and Audiology Canada. She has also completed the Level 1 Neurorehabilitation: Assisting Recovery and Function in Everyday Life Following Brain Injury course through Brock University.

Tasha Sweitzer

Tasha Sweitzer

Tasha is a Registered Nurse and Rehabilitation Case Manager with several years of nursing experience throughout the acute care, long term care, palliative and community and rehabilitation settings.  Tasha spent years working as a clinical manager and a clinical educator in the community, and as a Clinical Nursing Instructor with the Faculty of the Algonquin College Health Sciences Team.

Tasha’s vast knowledge and previous history of accessing community resources for her clients throughout a variety of sectors and funding streams, aids in the delivery of care excellence within the role of case manager.  Proficient in assessing, planning, implementing, and evaluating the clinical and rehabilitation needs of individuals with acquired brain injury, spinal cord injury, orthopaedic injuries, chronic pain, and mental health disorders,  Tasha delivers care with compassion and understanding and offers a holistic approach to finding the best resources available for the client's individual needs.  

Tasha is a member in good standing with the College of Nurses of Ontario (CNO) and the Registered Nurses Association of Ontario (RNAO).

Angela Van Hofwegen

Angela Van Hofwegen

Angela is a Registered Nurse and Rehabilitation Case Manager with extensive experience in acute care nursing,  geriatric care, and community rehabilitation nursing.  She has also worked as a Clinical Nursing Instructor with the Faculty of the Algonquin College Health Sciences Team.

Angela has extensive experience in assessing, planning, implementing, and evaluating the clinical and rehabilitation needs of individuals with acquired brain injury, spinal cord injury, orthopaedic injuries, chronic pain, and mental health disorders.   Angela's  experience in case management offers extensive knowledge of how to access several community resources within a variety of sectors and funding streams and the governing legislation within those sectors. Angela is dedicated to offering care with both  compassion and understanding while offering a holistic approach in finding the best resources available for the client's individual needs.

Heather MacGregor

Heather is a Registered Occupational Therapist and Rehabilitation Case Manager with extensive experience providing community and clinical based occupational therapy assessment and treatment services to a variety of client populations, including acquired brain injury diagnoses, spinal cord injuries, orthopaedic, musculoskeletal, chronic pain and mental health disorders. Proficient in conducting in-home functional and home safety assessments; assessments of attendant care needs; hospital discharge assessments and discharge planning; functional cognitive assessments; Insurer’s Examinations; job analysis assessments and ergonomic worksite consultations and assessments. Heather is dedicated to establishing client-centered and evidence based occupational therapy treatment plans and working collaboratively with Clients to support goal attainment and re-engagement in daily activities.

Heather has knowledge and experience assessing the rehabilitation needs of others and assisting clients to access community resources within a variety of sectors and funding streams and the governing legislation within those sectors.

Heather is an Assistive Device Program (ADP) Authorizer for mobility aids, including walkers, wheelchairs, power wheelchairs, scooters and seating and positioning equipment.

Kristen Young

Kristen is a Registered Occupational Therapist and Rehabilitation Case Manager with extensive experience working with a variety of client populations, including, traumatic brain injury, spinal cord injury, musculoskeletal injuries, chronic pain and mental health diagnoses. She has experience working in the community and in a clinic-setting completing assessments (Attendant Care Needs, in-home functional, home safety, functional cognitive, physical demands analysis and ergonomic worksite) and treatment or consultation. Treatment with Kristen focuses on establishing a therapeutic relationship, working towards client-centred goals through collaboration with the client and other treating health professionals while using an evidence-based approach to re-engage in meaningful activity. Kristen has been working with clients in a variety of sectors (auto-insurance, WSIB, Veterans Affairs Canada and private) and is familiar with funding and legislation within these sectors.

Kristen is an Assistive Device Program (ADP) Authorizer for mobility aids, including walkers, wheelchairs, power wheelchairs, scooters and seating and positioning equipment.

She is a member in good standing with the College of Occupational Therapists of Ontario (since 2010) and the Canadian Association of Occupational Therapists.

Jennifer Hicks

Jennifer (she/her) is a registered Speech-Language Pathologist and Rehabilitation Case Manager. For the past 25 years she has been part of multidisciplinary neuro-rehabilitation teams on in-patient units, in out-patient settings, within the community and in private practice. Jennifer is a Clinical Educator and guest lecturer at University of Toronto - Department of Speech-Language Pathology. She enjoys her volunteer roles with Speech-Language and Audiology Canada as a mentor in the Career Development Program and as a member of the Equity Diversity & Inclusion Committee.

Jennifer takes a comprehensive holistic approach toward her clients and creates tailored client-specific plans throughout the rehabilitation process. Her compassion, resourcefulness and network of community supports in a variety of sectors help her clients optimize their recovery.

Jennifer is a member in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario and of Speech-Language and Audiology of Canada.

Maegan Whitteker

Maegan is a Registered Occupational Therapist and Rehabilitation Case Manager with several years of experience providing clinical and community based occupational therapy consultation, assessment and treatment services to a variety of client populations, including individuals with acquired brain injuries, amputations, spinal cord injuries, orthopaedic injuries, musculoskeletal injuries, chronic pain, and mental health disorders. She is experienced in conducting attendant care needs (Form 1) assessments; in-home functional assessments; home safety and accessibility assessments; hospital discharge assessments and discharge planning; functional cognitive assessments; seating and mobility assessments; Insurer’s Examinations; and job analysis and ergonomic worksite consultations and assessments in a variety of settings. Maegan has been working with clients in a variety of sectors including auto-insurance, Workplace Safety and Insurance Board, Veterans Affairs Canada, and private. Maegan works collaboratively with her clients, their families and other rehabilitation team members to provide client-centered and evidence based occupational therapy treatment to support goals and re-engagement in meaningful daily activities.
